    Cardiac surgery in Israel

    Cardiology and cardiac surgery medical centers in Israel are popular all over the world, as they are equipped with innovative equipment and robotics devices, and only highly qualified, world-famous specialists work here. Department of Cardiac Surgery at Tel Aviv Medical Center is no exception. Every day, lives are saved here and patients from severe heart and vascular diseases are cured.

    In cardiac Department of Cardiac Surgery of TAMС, patient can receive all the necessary medical care; there are practically no treatment methods that are not practiced by cardiac surgeons in our department, be it classical open-heart surgery or minimally invasive percutaneous modern interventions. The department even has a unique hybrid operating room, where combined interventions are performed – open heart + catheter percutaneous manipulations.

    Departments

    On the basis of Department of Cardiac Surgery there are several divisions that deal with their specific cardiac surgery areas:

    • Department for diagnostics and treatment of cardiac arrhythmias – all necessary diagnostic procedures are carried out here, including complex electrophysiological studies, as well as all types of minimally invasive treatment of identified disorders, for example, RFA (radiofrequency ablation) of arrhythmia source.
    • Department of Aortic Surgery – cardiac surgeons in this area wiil perform surgeries on aorta if it is diseases, for example, dissection or aneurysm.
    • Department of Interventional Cardiology – minimally invasive percutaneous catheter interventions on heart are performed here, for example, balloon angioplasty and stenting of coronary arteries, catheter installation of an aortic valve prosthesis, etc.
    • Cardiac intensive care unit is an intensive care unit where patients with acute lesions of heart and blood vessels (myocardial infarction, aortic aneurysm dissection, etc.) receive the necessary treatment, as well as patients after cardiac surgery for some time.
    • Open Heart Surgery Department – specialists in this area skillfully perform the most complex cardiac surgeries that require open access to heart and often connection to a heart-lung machine (coronary artery bypass grafting, plastic and prosthetic heart valves, heart transplantation).
    • Diagnostic department – here there are all available methods for cardiovascular system diagnostics: ECG, heart ultrasound, stress tests, blood laboratory, coronary angiography, cardiac MRI, myocardial scintigraphy and much more.
    • Department of Robotic Cardiac Surgery – specialists in this area perform surgeries on heart and blood vessels using innovative robotic installations, for example, Da Vinci robotic surgeon.
    • Department of Vascular Surgery – deals with diagnostics and treatment of diseases, injuries and defects in arteries and veins development.
    • Cardiac rehabilitation department – here patients undergo the recovery stage of treatment after surgeries on heart and blood vessels.
    • Department of Cardiothoracic Surgery – surgeons in this area are engaged in surgical treatment of diseases, injuries and development defects of chest organs (bronchi, pleura, lungs, mediastinal organs).

    What diseases does Cardiac surgery Department treat?

    Diagnostic methods in cardiac surgery

    Diagnostic techniques that are used to examine cardiac patients:

    • a set of necessary laboratory blood tests (complete blood count, heart disease markers, extended lipid profile, etc.);
    • overview digital radiography of chest organs;
    • computed tomography of chest with and without contrast;
    • echocardioscopy, or ultrasound of heart, with color Doppler sonography, transesophageal echocardiogram (TEE);
    • electrocardiography and Holter ECG monitoring;
    • changes in blood pressure and daily blood pressure monitoring;
    • MRI of heart;
    • myocardial scintigraphy with Tl and Tc isotopes;
    • stress tests (treadmill, step test, bicycle ergometry), including drug provocative tests;
    • coronary angiography;
    • CT angiography (virtual angiography);
    • aortography;
    • Doppler ultrasound of peripheral vessels (arteries and veins);
    • Electrophysiological study of heart.

    Important!

    Department of Cardiac Surgery at TAMC has all modern and classical methods for diagnosing cardiovascular diseases, because in order for treatment to be effective, a correct diagnosis must be made.

    Only after a thorough and in-depth diagnostic program is a decision made about the need for cardiac surgery.

    In addition to commonly used methods for cardiovascular system studying, Department of Cardiac Surgery at Tel Aviv Medical Center also practices innovative diagnostic procedures. An example would be IVUS procedure – intravascular ultrasound. Unfortunately, in some cases, coronary angiography provides information about luminal characteristics of vessels, but does not provide a clear picture of pathological changes in vessel.

    However, with IVUS guidance, doctor will be able to see a clearer picture of the disease and improve patient outcomes. IVUS is a method of ultrasound examination of coronary vessels from the inside. To do this, a miniature ultrasound sensor is brought to coronaries using a catheter inserted through a peripheral vessel. IVUS system includes a central processor, control console, monitors, patient interface module, and patient table controller. The system is installed and operates in angiography laboratories.

    Modern treatment methods in cardiac surgery

    In Israel cardiac surgery is constantly developing and improving. Every year, clinical trials of new drugs and surgical techniques for treating cardiac patients are carried out at the departments.

    At Department of Cardiac Surgery of Tel Aviv Medical Center patients receive access to all the latest innovations in the field of surgical treatment of cardiac patients. For example, people with diastolic chronic heart failure are fitted with CORolla device by CorAssist Cardosovascular company, developed in Israel and approved for use in medical practice.

    Diastolic type of heart failure – is when, due to a certain disease (often arterial hypertension), heart chambers cannot completely relax and fill with required amount of blood, which causes a decrease in cardiac output and symptoms of heart failure. TAMC cardiac surgeons implant a kind of elastic spring into the cavity of left ventricle through minimally invasive percutaneous catheter intervention. The device dilates ventricular cavity and eliminates the symptoms of diastolic heart failure. This is the first such device in the world designed to treat this type of heart failure.

    Important!

    As is known, cardiovascular pathology is the main cause of premature mortality throughout the world, but over the past 10 years this number in Israel has been significantly reduced.

    Cardiology and cardiac surgery are considered a priority medical area for the state, and therefore are fully funded. This allows us to actively develop and quickly implement new and highly effective methods for diagnosing and treating cardiac diseases.

    Mitral and aortic valve repair by cardiac catheterization. Until recently, any manipulation of heart valves required open access to organ; now in modern cardiac surgery departments such rurgeries are performed through a minimally invasive cardiac catheterization procedure. Surgery does not even require general anesthesia; all stages of intervention are performed under local anesthesia through percutaneous access, for example, through femoral artery.

    Coronary artery bypass grafting is surgery that restores normal blood flow in coronary arteries when they are damaged, for example, by atherosclerosis. Specialists at TAMC clinic successfully use various types of coronary artery bypass grafting to restore blood supply to heart muscle, using both classical open-heart technique with artificial circulation and various options for minimally invasive bypass interventions.

    TAVI surgery (transcatheter aortic valve implantation) is a minimally invasive heart surgery to replace a narrowed aortic valve that does not open properly. The intervention can be performed consciously under local anesthesia or general anesthesia. Through a small hole in femoral artery, valve is rolled up and delivered to heart, where it is installed. Once the new valve is in place, balloon at the tip of catheter is inflated to deploy replacement valve. The procedure is carried out under X-ray control. When surgeon is confident that the valve is securely attached, catheter is removed.

    For patients who need open heart surgery, treatment in Israel can be an excellent chance for recovery and a subsequent full life. At TAMC, such interventions are carried out using modern surgical techniques and innovative equipment, so even extremely complex interventions, for example, pacemaker installation, repair of an aortic aneurysm, and even heart transplantation, are completed successfully and with the least complications.
