    Hemicolectomy

    Hemicolectomy – removal of half of the colon; from the terminal ileum to the middle of the transverse colon – right, from the middle of the transverse colon to the rectosigmoid (rectum) – left. In the leading Israeli clinic Tel Aviv Medical Clinic, hemicolectomy is also performed using video endoscopic surgical technologies. The use of such techniques can minimize the length of hospital stay, reduce the frequency and severity of complications, reduce trauma and get a good cosmetic effect.

    When surgery is prescribed

    Hemicolectomy is performed in the case of the following diseases:

    • colon cancer – depending on the location of the neoplasm, distinguish between right and left hemicolectomy. The complexity of the operation is associated with the nature of the blood supply to the intestine, the need to remove a large number of regional lymph nodes and the greater strength of the anastomosis applied to the transverse colon;
    • intestinal obstruction;
    • intestinal polyposis;
    • Crohn’s disease.

    Contraindication is due to various reasons (stage and severity of the course of cancer, the patient’s serious condition and others) inoperability.

    Treatment

    Hemicolectomy on the right – the operation is performed both laparoscopically and using laparotomy. During laparotomic surgery, a median incision of the peritoneum is performed, the abdominal cavity is carefully examined for pathologies. After this, the ligaments, main blood vessels, mesentery and intestinal loops are sequentially ligated and dissected. Then the next stage of the procedure begins – the formation of an anastomosis between the terminal portion of the ileum (thin) and colon (colon). The final stage of the operation is closing the hole in the peritoneum and layer-by-layer suturing of the anterior wall of the peritoneum.

    Left-sided hemicolectomy, technique – can be carried out by laparotomy (open) and laparoscopic (closed) methods. In abdominal laparotomy, a midline peritoneal incision is made. Laparoscopic left-sided hemicolectomy is performed through small incisions of the abdominal wall, through which instruments and a video camera are inserted into the abdominal cavity, transmitting the image of the surgical field to the monitor. Affected and adjacent areas of the colon are resected, an intestinal anastomosis is applied. In some cases, removal of the intestinal stoma is required. The duration of the procedure is 1.5-3 hours.
