Diagnostic methods in urology
As a rule, the following examinations are used in combination:

- General clinical examinations of blood and urine, if necessary – other specific examinations of blood and urine, for example, determination of PSA (prostate-specific antigen – a tumor marker for prostate cancer).
- X-ray (including with contrast of urinary system), CT, MRI, PET-CT, MSCT with and without contrast – methods of detailed medical imaging in urology.
- Scintigraphy for diagnosis of distant metastases in malignant diseases of excretory and reproductive system.
- Ultrasound of kidneys, prostate, bladder, ultrasound of blood vessels.
- Biopsy and histological examination of collected species to verify the diagnosis.
- Cystoscopy, ureteroscopy – examination of organs from inside using endoscopic techniques.
If necessary, a biopsy or treatment procedure is immediately performed, for example, removal of a polyp in bladder or a calculus from the ureter, etc.

The 4KSCORE blood test makes it possible to assess the risk of developing aggressive prostate cancer in patients suspected of developing this disease. Previously, a total PSA (prostate-specific antigen) test was used for this purpose, but this marker is not specific for prostate cancer, so an unnecessary invasive prostate biopsy procedure was often prescribed. The 4KSCORE analysis makes it possible to determine this risk much more accurately and avoid unnecessary biopsies. This is a set of 4 biomarkers that are determined in blood. Interpreting the result, doctor takes into account age and other risk factors for prostate cancer. If the risk is calculated to be low, a biopsy is not prescribed. According to recent data, due to introduction of such screening instead of usual PSA determination, 65% of unnecessary biopsies were avoided.
FUSION prostate biopsy is a modern technique for obtaining histological samples of prostate tissue. The technology combines two examination methods – transrectal ultrasound and MRI. It allows us to take more samples of biological material from hard-to-reach areas of prostate gland, which improves chances of detecting a malignant neoplasm. The accuracy of examination increases from 75% with conventional ultrasound-guided biopsy to 95% with FUSION technique.
Gallium 68 (Ga-68) PSMA PET-CT is a modern method of medical imaging for prostate cancer, the presence of metastases in this disease and monitoring prostate cancer recurrence. The enzyme PSMA (prostate-specific membrane antigen), labeled with gallium 68, actively accumulates in malignant cells in prostate cancer. Thus, PSMA PET-CT with gallium 68 makes it possible to identify even the smallest number of such cells and their location in body.
Modern treatment methods in urology

Open removal of prostate is used in the most difficult cases, when it is impossible to remove a prostate gland damaged by a malignant tumor laparoscopically.
Urology Department actively uses modern treatment methods for erectile dysfunction in men. These are procedures such as injection therapy with Trimix and Biomix, shock wave therapy, PDE5i treatment (phosphodiesterase type 5 inhibitors).
If necessary, men can undergo a penile prosthesis procedure – installation of penile implants. This is a surgical method of restoring an erection by implanting a special prosthesis inside phallus.
The clinic uses the most modern drug treatment regimens for prostate cancer when surgery is contraindicated or complex treatment regimens are used. One of these is GnRH agonists or antiandrogens application. This is one of the types of hormone therapy that is used when patient has hormone-dependent tumors. The main aim of hormone therapy is to suppress the action of androgens in a man’s body, slow down the division of cancer cells, change their function so that they die on their own and no longer reproduce.
In complex treatment of malignant neoplasms of prostate, a type of radiation therapy is used, such as prostate brachytherapy. This is when a radioactive isotope in a capsule is placed directly into tissue of a malignant prostate tumor, while the surrounding healthy tissue is not affected by ionizing radiation.
