    Free Prostate-Specific Antigen (Free PSA) represents the unbound fraction of the protein circulating in the blood independently. Its presence in the systemic stream depends on the secretory activity of the prostate epithelium and the intensity of its cell renewal. In clinical diagnostics, this parameter acts as an important clarifying indicator. The ratio of the free form to the total amount allows physicians to assess the biological activity of prostate tissues in greater detail and distinguish functional fluctuations from structural processes.

     

    What does the analysis represent?

    • Purpose: Measurement of the quantitative content of the free PSA fraction in blood serum.
    • Primary Application: Detailing total PSA results, calculating fraction ratios, monitoring prostate tissue status, and assessing therapy.
    • Method: High-tech laboratory analysis of a venous blood sample.

    Recommendations for the Test (General)

    • Timing: Blood collection is best planned for the first half of the day.
    • Fasting: Observe a “fasting window” of 4 to 6 hours.
    • Rest: It is important to spend 20–30 minutes in a relaxed, seated position before entering the room.
    • Avoidance: For 48 hours before the test, exclude sexual contact, cycling, and heat exposure (saunas).
    • Medications: Inform the physician about any urological treatments or biotin-containing supplements.

    What can affect the results?

    • Factors increasing levels: Prostate inflammation, benign prostatic hyperplasia (BPH), recent prostate massage, or biopsy.
    • External influences: Catheterization, cystoscopy, or intensive physical exertion on pelvic floor muscles can distort the figures.

    When to take the test?

    • Recommendations: Ordered as an additional step for borderline total PSA values or to monitor the dynamics of previously identified conditions.
    • Preparation: Ideally taken simultaneously with total PSA to obtain a correct calculated ratio.

    How to interpret the results? 

    Numerical interpretation is a professional task for the urologist. The Free PSA value is never viewed in isolation. The specialist analyzes it only in conjunction with the total PSA level, calculating their percentage ratio. This evaluation requires considering age, gland volume, and TRUS or MRI data. Self-diagnosis is unacceptable.

     

    Possible further investigations

    • Expanded Calculations: Determination of the Prostate Health Index (PHI) if necessary.
    • Visualization: Multiparametric MRI or TRUS to clarify organ structure.
    • Urologist Consultation: Follow-up exam to correlate all data for a final conclusion.

    When does the next step make sense?  

    If a Free PSA test is ordered due to borderline total PSA levels, results are viewed holistically — with age, total PSA levels, and imaging data. The next steps are always decided individually after medical evaluation.
