    Legionella Ag Urine Test

Detects particles of Legionella bacteria in urine.

    Result: 1-3 days Code: 87278
    613.50

    Description of the urine test:

The Legionella Antigen test is a high-speed diagnostic tool used to detect Legionella pneumophila (specifically serogroup 1), the bacterium responsible for a severe form of pneumonia known as Legionnaires’ disease. Unlike blood tests that detect the body’s immune response over time, this test looks for specific bacterial proteins (antigens) excreted in the urine. These antigens can often be detected as early as 1 to 3 days after the onset of symptoms, making it a critical “gold standard” for rapid clinical intervention in cases of acute respiratory distress.

    What does the analysis represent?

    • Purpose: Qualitative detection of Legionella pneumophila serogroup 1 antigens in a urine sample.
    • Primary application: Early diagnosis of atypical pneumonia, differentiation of respiratory infections, and investigation of sudden high fever with pulmonary symptoms.
    • Performance: Immunochromatographic assay or ELISA of a urine specimen.

    Recommendations for the test (general)

    • Time: The sample can be collected at any time of the day.
    • Container: A sterile plastic container must be used for collection.
    • Stability: Antigens often persist in urine even after the initiation of antibiotic therapy, allowing for an accurate diagnosis during the treatment phase.
    • Important: This test specifically identifies Serogroup 1, which accounts for the vast majority of community-acquired Legionnaires’ disease cases.

    What can affect the results?

    • False-negative results: May occur if the infection is caused by a different Legionella serogroup or species, or if the urine sample is extremely dilute.
    • Timing: While antigens appear early, testing within the first 24 hours of symptoms might occasionally yield a negative result; repeat testing may be necessary if clinical suspicion remains high.

    When to take the test?

    • Recommendations: The test is vital for patients presenting with high fever, a dry cough, shortness of breath, and gastrointestinal symptoms (like diarrhea), especially if they have been exposed to large-scale water systems or air conditioning units.
    • Preparation: No specific dietary restrictions or fasting are required for this urine-based study.

    How to interpret the results?

The interpretation of data is the exclusive responsibility of a pulmonologist or an infectious disease specialist. A positive result for the Legionella antigen is a clinical indicator of an active or very recent infection but is not a standalone diagnosis of the disease’s severity. The physician correlates this result with chest X-rays and the patient’s respiratory status. Only a professional can confirm the diagnosis of Legionnaires’ disease and immediately prescribe the specific antibiotic therapy required to treat this aggressive pathogen.

    Possible further research

    • Chest X-ray or CT Scan: To assess the extent of pneumonia and lung involvement.
    • Complete Blood Count (CBC): To monitor the systemic inflammatory response.
    • Sputum Culture: To attempt to isolate the bacteria for antibiotic sensitivity testing.
    • Consultation with a Pulmonologist: For specialized management of acute respiratory health.

When does the next step make sense?

A positive result is a medical priority. Because Legionella pneumonia can progress rapidly, the next step—a consultation with a specialist—must happen immediately to initiate life-saving treatment and monitor for potential complications.
