This test measures the time it takes for thrombin to convert fibrinogen into fibrin strands. By isolating this specific reaction, the analysis can detect issues related to fibrinogen structure or the presence of inhibitory substances that interfere with this transition. It serves as a focused diagnostic signal when broader clotting screens indicate an abnormality that needs precise clarification.

     

    What does the analysis represent?

    • Purpose: Assessing the efficiency of the conversion of fibrinogen to fibrin.
    • Primary Application: Identifying fibrinogen abnormalities and detecting the presence of specific anticoagulants.
    • Method: Measuring the clotting time of plasma after the addition of a standardized thrombin reagent.

    Recommendations for the Test (General)

    • Timing: Best performed in the morning for consistent results.
    • Fasting: A fasting pause of 8–12 hours is required.
    • Rest: Remain in a seated, calm position for 15–20 minutes before the blood draw.
    • Specific Rules: It is vital to list all current medications, especially any blood thinners, before the test.

    What can affect the results?

    • Factors altering levels: The presence of heparin in the sample, abnormally high levels of fibrin degradation products, and rare hereditary disorders of fibrinogen.

    When to take the test?

    • Recommendations: Ordered as a follow-up to a prolonged PTT or when a physician suspects a specific problem with the quality of a patient’s clotting proteins.
    • Preparation: Best performed when the patient is not on active heparin therapy unless being used for monitoring.

    How to interpret the results? 

    Data interpretation is the exclusive responsibility of a physician. A prolonged Thrombin Time indicates that something is delaying the final step of clot formation. The physician must differentiate between a physical lack of protein and the presence of a blocking substance. Only an expert can correlate this timing with other factor assays to determine the underlying cause of the delay.

    • Reptilase Time: To distinguish between heparin influence and fibrinogen defects.
    • Fibrinogen Antigen Test: To measure the physical amount of the protein present.
